Iron Rx Physical Therapy Athlete Sponsorship Terms
Iron Rx Physical Therapy chooses to sponsor one male and one female athlete through their rehabilitation process and return to sport.
This sponsorship is designed to help an athlete with an injury who otherwise is unable to receive the highest level of care they deserve for any given reason. Being sponsored means that Iron Rx Physical Therapy will provide the selected athlete with the highest level of appropriate care for their given injury and goals without any financial responsibility on the athlete, athlete's parent(s), or athlete's legal guardian(s). This sponsorship will last for a period no greater than 9 months.
In order to participate in the sponsorship program, the athlete(s) or if under the age of 18, their parent(s) or legal guardian(s) must agree to the following terms:
1. The athlete must be present and on time for all scheduled therapy sessions. If a conflict arises, the session must be rescheduled at least 24 hours in advance. If the session is not rescheduled more than 24 hours in advance or the athlete misses their scheduled session, the athlete, parent(s), or legal guardian(s) will be charged for the price of the session ($189).
2. The athlete, parent(s), or legal guardian(s) agrees to be photographed, filmed, audio recorded, and/or interviewed for promotional purposes and used by Iron Rx Physical Therapy and it's partners. These photos, videos, audio clips, and/or interviews may be used on Iron Rx Physical Therapy's various platforms including social media, website, YouTube, email, mailings, and other promotional forms of print media.
3. The athlete, parent(s), or legal guardian(s) agrees to post on their personal social media platforms at least once per week to promote Iron Rx Physical Therapy. It is asked that Iron Rx Physical Therapy be tagged in these posts not only to monitor that they are being done, but also so that they can be shared on the Iron Rx Physical Therapy social media pages as well.
Examples of this may be in-session pictures or videos, videos or pictures of home exercises, videos or pictures of athlete or family member wearing Iron Rx Physical Therapy apparel, etc. Note: This does not include checking-in on Facebook which is discussed below.
4. The athlete, parent(s), or legal guardian(s) will "Check-in" on Facebook for each scheduled therapy session.
5. The athlete, parent(s), or legal guardian(s) will positively endorse Iron Rx Physical Therapy to friends, family, teammates, coaches, and others. As part of this, the aforementioned person will provide Iron Rx Physical Therapy with a potential client referral at least once per month.
6. By completing the form below, and in consideration for participation in the Iron Rx Physical Therapy Athlete Sponsorship, I do waive and forever release any and all rights and claims for damages and liabilities of any kind arising out of my participation in this sponsorship, against all persons, entities, and agencies involved with the treatment sessions, including, but not limited Iron Rx Physical Therapy, and any other related individuals or entities.
